How the Referral Process Works

Call our office at (833)338-6831 or fax referral documents to (361)298-2229. Our team is available 24/7 to accept referrals.

Provide the patient's recent office visit notes, lab work, H&P, face sheet, current medications, and order to eval and treat for Hospice Care.

Our intake team will contact the patient and family within hours, verify eligibility, and coordinate a seamless transition to hospice care.

An interdisciplinary team — nurse, aide, social worker, and chaplain — is assigned and care begins, often the same day as the referral.

Anyone who believes a patient may benefit from hospice care is welcome to reach out. Referrals come from many sources:

A patient is generally eligible for the Medicare Hospice Benefit when all of the following apply:

A terminal diagnosis with a prognosis of six months or less if the illness runs its normal course
A physician certifies the patient is eligible for hospice care
The patient (or legal representative) elects the hospice benefit
The patient agrees to comfort-focused care rather than curative treatment
